TO REGISTER YOUR CHILD FOR A LACROSSE TEAM, TAKE THE FOLLOWING STEPS:
1) Register with Shelburne. This can be done online at the Parks & Rec Website. Or you can obtain a registration form from Shelburne Parks and Recreation Department by printing out a registration form from the DOCUMENTS section of this web site. The form is titled YOUTH LACROSSE REGISTRATION FORM. In the alternative you can pick one up at the Shelburne Parks and Recreation Office or print one off from the Shelburne Parks and Recreation web site.
2)For the Boys 3/4, 5/6, or 7/8 team and Girls 5/6 or 7/8 team visit the US Lacrosse web site (click on the link to the left) and either renew your child's US Lacrosse membership or have your child become a member of US Lacrosse if you have not done so already. Record your child's US Lacrosse number and expiration date (you will receive a confirmation e-mail with this information shortly after becoming a member/renewing) and place them on the registration form. (Your child must have valid US Lacrosse membership through June 30, 2013). If you do not have internet access or do not want to pay by credit card, contact Shelburne Parks and Recreation for assistance.
Note-You will need your child's US Lacrosse membership number to renew his membership. If your child has been a member of US Lacrosse and you don't remember his/her US Lacrosse membership number, you can find it on his US Lacrosse membership card, on the mailing label of Lacrosse Magazine, or by looking it up on the US Lacrosse web site (click on the link above and look for the line "look up online.")
3)Complete the online registration or the registration form and return the form and payment (see below for registration fees) to Shelburne Recreation. You can do this by either bringing the form to the Shelburne Recreation office, mailing it to Shelburne Recreation at the address below, or faxing it at the fax number below. (Faxed registrations must have a check sent by mail). For Boys and Girls 3/4, 5/6, and 7/8 you will need to include your child's US Lacrosse membership number and expiration date on the registration form.
NOTES- Make Checks payable to Shelburne Recreation. There will be no confirmation of registration sent. Once you submit the completed registration form and send in the appropriate fee, consider your child registered.
